🚨ELDER NEGLECT WARNING🚨 Do not send your loved one here. My mom was there for less than 24 hours. She experienced neglect. I called 911 and got her out. Conditions are terrible. It’s not a real medical facility. Mom arrived from hospital transport with a broken sacrum and fractured bones. She was not given pain medications. No one checked on her. 💔 She was placed in an unsupported, sunken bed. She lay there paralyzed for hours! She managed to call me with her cell phone. 911 was excellent! We saved her life tonight by getting her out of there. Without taking that step, she probably would not have survived the night. I will need therapy after this. My heart is broken. I feel for the other residents. If you’re thinking of this facility, don’t rely solely on the reviews. Visit in person an

While the facility is not new, the food heavy on starches, and communication between staff could be improved, this facility gets 5 stars for thing that really count. Los Alamos Wellness and Rehab was able to find a place for my elderly parent on very short notice. The staff was competent at filing and getting approval for skilled nursing rehab from her health insurance. Rehab (PT and OT) went well and my parent was able to return to assisted living in a little over two weeks. The staff was very helpful with discharge and ensuring my parent had the necessary medical equipment. The rose garden was a standout safe space for patients wanting to enjoy some nature and outdoor time.

Los Alamos wellness and rehabilitation deserves a five star rating. From the moment we entered meeting Monica, who was very knowledgeable and answered almost all the questions before we asked them. There were so many nice people. It’s just unbelievable that all the people there can be so caring. They were always asking us if they could help us or how they could help us. The OT and PTA were very knowledgeable in their profession. They provided us with valuable information and instructions for exercising to improve our health. We have 24 names of people who were there to help us through our rehabilitation. They all were very kind. They have a very nice garden with a rail all the way around the garden which made it perfect for exercising and enjoying the outdoors while recuperating. The food

We would like to Thank Monica for helping us with my Dad. She helped us with the process and made the transition smooth and worry free. We would also like to thank Jenny for the excellent care she has given my father during the hard transition he has had and also Nurse Pauline for being so informative and responsive. We would also like to Thank Elana and Holly in Physical Therapy for there continued work with him and staying so friendly and upbeat while working with him.

My mom was enrolled at Los Alamos Wellness and Rehabilitation. She was treated with so much kindness and respect. The staff was respectful and professional. They treated us all like family! Everytime I left, I knew my mom was in good hands! I knew she was safe! Thank you to everyone who made her stay a comfortable one. We appreciated you! Rita
