I can no longer give a good recommendation because I simply need a doctor to do a physical and fill out the form that I have, but without even looking, he gave me an instant no, while I enjoy my job giving 1 visit a year and having to only pay 60 which isn't a bad price, my forms require a doctor license, it doesn't mean they have to know me personally and I even have to sign release giving permission to let them treat me on each form
