South Mountain Post Acute

I can not rave enough about this facility! Clean, organized & the staff is so friendly, patient & most importantly goes the extra mile for each resident! The residents were smiling ear to ear & looked well cared for! 5 stars!

Hi! I just wanted to say that I've had great experience with the therapy team here at South Mountain Post Acute. When I first came here I could not speak but my wonderful Speech Therapist, Nicole, has helped me relearn to speak, read, and write. Brent, Eli my Physical Therapists have helped me learn to walk and run again! They are so wonderful! Zak, my Occupational Therapist has been an advocate for me and helped me become more independent. But food here is terrible!

Hello I'm currently a patient my name is Georgina Herrera , I have been here for going on 2weeks already and the staff here they are so amazing and sweet . There are a handful of nurses I want to give a shot out too for being so caring and patient : Edna Akerele, Leslie, Bernard, Yusbi, Evelyn, and special shot out to my activity's nurse for always having a smile and keeping me busy her names is Asianae. And not forgetting Delisa(Delicious) . Woundcare :Jenifer. Nightshift: Lina, and Bernard, the new girl Marie. In this fast paced of Technology sometimes we need to just sit down and reflect the people who are taking care of you. I like to go ahead and explain my situation I had Surgery which got infected. I was transfer to a place called South Mountain Post Acute My options are vegan to help my healing process. The management and staff went above and Beyond to accommodate me . This is why I elect to give out some really positive kudos to three amazing workers and they are . Mr. James Hunter. Miss Keyondra Lane and Mr. Jesus Espinoza thank you for all you've done for me. The Dietary staff has assisted me in my recovery having a successful healing process the food was great. Protein, calories dietary efficiency It's always hot on time. The staff is courteous professional extremely approachable.

***BEWARE: MEDICAL NEGLECT & INCONSISTENT CARE*** I'll start with the little bit of good we experienced before our loved one was transferred to ICU from this place. Mrs. Iris actually cares for people and it shows in how she handles her patients and the physical therapy helped to challenge our loved one to become more self-sufficient while there. Other than these two, there is NOTHING positive about the harm, injury, and gross negligence experienced by us toward our loved one. Preface: Our loved dedicated almost 45 years of life as an LPN in skilled nursing facilities such as this one and about 10 years as an aide prior to becoming a LPN.
