Phoenix Mountain Post-Acute

I never want anyone to go through what I did with my seizure three weeks ago, but I am so grateful that Honor recommended Phoenix Mountain Post Acute. I’m a very independent person, so at first, I didn't like how 'attentive' everyone was—they are always right there to help! But the PT and Occupational Therapy teams gave me the exact exercises I needed to feel like myself again. Things I couldn't do two weeks ago, I can do now. The nurses, the CNAs, the cooks, and the case managers all go above and beyond. Even with my aphasia, I saw how they keep everyone engaged and cared for. My 2-week stay turned into only 9 days because of their help. When you’re in a crisis, you want the best. This place is 100% 5 stars. Every day is a gift!

My family had a heartbreaking and deeply disappointing experience with Phoenix Mountain Post Acute. From the moment my grandmother arrived, the level of care and attention felt extremely lacking. For a rehabilitation facility meant to care for elderly patients, compassion, urgency, and consistent support did not feel present. The most painful part was at the end of her life. My grandmother became unresponsive the night before she passed, yet our family and hospice were not contacted when this was first noticed. We only realized something was seriously wrong after calling the nurses’ station ourselves and immediately rushing there, leaving us with only a few short hours before she passed in front of us. Wonderful facility, very nice staff, amazing nurses and just an overall wonderful experience! The activities Phoenix Mountain is more than I’ve seen at other facilities, my favorite is the Puppies on Wheels! ❤️

I would like to thank phoenix mountain for the excellent care I received. From the moment I arrived the staff didn't care for me they cared about me and treated me like I was their family. The cnas and Nurses were so compassionate and responsive to my needs. The therapy team helped in my recovery. Their patience and constant encouragement kept me going during my rehabilitation. The staff acted beyond clinical and treated me with real human empathy. Thank you for giving me a home away from home while I recovered.

Both my grandma and mother spent their final years at Phoenix Mountain. I can't say enough about how well they treated both of them. Loving and caring nurses, aides, and complete staff. I can highly recommend them as I've seen them in action providing wonderful care.
