One Heartland at San Antonio Wellness and Rehabilitation

Wonderful place! Everyone is friendly and there are lots of activities. They all take really good care of my mother, and she looks forward to spending time there. I'm so thankful that we have SA Wellness for our monthly respite visit.

This is my second time going there I can honestly say is ok facility the only reason I don't give them 5 stars is because of The Dietary Manager he was rude never got my food right...... But the nurses like Elsa and Rosa are great also I would like to say thank you to miss G she was always very nice and patience because I have strong personality .

I am extremely disappointed with the care my mother is receiving at this nursing home. Every time I visit, the facility is disgustingly dirty. The rooms reek of unpleasant odors, and on multiple occasions, I have witnessed fecal matter on the walls and curtains. It's appalling. Furthermore, there's a pervasive sense of illness among the residents. While staff may deny it, it's clear that many residents are sick. The nursing staff is unhelpful and unresponsive. On one occasion, I asked a nurse(the heavy set one) at the front desk for my mother's medication and assistance with changing her, only to be told that it was the CNA's job and that the nurses don't assist the CNAs. The housekeeping here is a joke. They merely give the impression of cleaning, but upon closer inspection, it's eviden

The Staff on the Memory Care Unit on Evening and Night Shift were very professional and cared for the patients well being. Had great attitudes and made sure patients were safe when walking about in the Unit.

Amazing small town community with family feel from the Nursing Staff to the Administrative Staff to the Dining Staff to Activities Staff to the Rehabilitation Staff to the Laundry and Housekeeping Staff. The Personnel truly care about each and every client and work to encourage them to live up to their best life. We have been clients for 3 years and are very happy living at San Antonio Wellness and Rehabilitation Center. Meals are homemade and plentiful and so are treats and a variety of activities.
