I would like to express my sincerest gratitude for the care my 92-year-old mother, J. Vera, received during her recent one-month stay at Legend Oaks for rehabilitation. I have to say that the facility is truly driven by its dedicated CNA staff. I want to give a special shout-out to Dee (CNA) for her exceptional care. Dee went above and beyond, taking over my mother’s care when we needed someone the most. She is truly amazing, and her dedication did not go unnoticed. I would like to applaud the Physical and Occupational Therapy ladies who worked with mom, you guys are TOP NOTCH!! Mom came out of rehab doing better than she was before her injury. My loved one has been a resident at Legend Oaks for a little over 2 weeks now. We visit her daily at different times of the day and can say that we are highly satisfied with the level of care and concern for the residents. The facility is comfortable, welcoming and clean. The staff gives residents the attention and are consistently engaging. Therapy is given consistently which is crucial for the healing process. The case manager Linda , an LVN. is professional, curtious, extremely knowledgeable and is clearly committed to the care of the residents. The moment you walk in you're met with warm greetings. I don't have many experiences with other Nursing facilities but am aware of the reputations and horror stories however, Legend Oaks is quite the opposite. I am satisfied with our decision

My father was a resident here for only a short time. He got admitted late in the evening and my sisters and I were not able to be there, so we showed up early the next morning. When we walked into his room we were pleased to see his nurse Ari already answering questions for him, getting him blankets and filling his water jug. She answered our questions multiple times either in person or on the phone. Her heart and compassion as a nurse really shined through and we were comfortable with his care when she was on shift. The CNAs were attentive and knew what they were doing. Our dad never had to wait long to be put to bed or taken to the bathroom. Juan is outstanding. I tried for almost a month to get my father placed here and was met with total admissions chaos. After receiving instructions to submit a clinical packet around the end of December, the referring facility promptly sent it over. There was no acknowledgment, no follow-up, and no communication. Calling the admissions department was nearly impossible. Most of the time the receptionist would transfer me, the phone would ring endlessly, and then the call would just hang up. I later found out the admissions staff member handling the referral (Angelica) was suddenly out for an indeterminate amount of time and did not hand the case off to anyone. When I finally reached someone else (Desiree), she had me resend the entire clinical packet. At that point we were three weeks in and starting from zero

My cousin has been in and out of this facility and every time he is here I am so happy that he is in good hands! The staff is always so happy and helpful they don’t seem bothered to help. It’s clean, they are prompt to help patients. My cousin enjoys the food, the whole staff are very helpful! Thank you for taking such good care of my cousin!
