There is something truly special about this place. This time last year, my grandmother was not doing well at all. While she could still recognize me, she was unable to dress or feed herself and often would not eat. After my grandfather could no longer provide the level of care she needed and with me living too far away and working long hours she was placed here. At that time, it felt as though she had lost her light. Since being here, that light has returned. She is now eating, participating in activities, taking herself to the bathroom, and even choosing her own clothing again. She has regained strength and confidence, and what once required significant assistance has turned into her walking the halls independently with her walker. 4 Oaks Healthcare Center is a clean, safe, and welcoming place. The nursing staff are friendly, caring, and always helpful. Patients look happy and well cared for, and the therapy team is outstanding—professional, knowledgeable, and focused on helping people reach their goals and return home. Highly recommend for anyone looking for quality care and rehab services!

Tell me why in God's name there was a different patients hardened feces in my grandmothers bedding. It wasn't hers and my grandfather picked it up not even knowing what it was because it was so old and dry. He handed it to the nurses and they held it (no gloves) and had no clue what it was till he explained. As someone in healthcare this is honestly the nastiest thing I've seen someone do. We have her in her because grandfather can't take care of her 24/7 but still has to go over there for most of the day just to make sure they don't leave her soiled or just left in the floor for hours after a fall. I don't understand why people would go to college, get a job in a nursing home, then not even do their job. I am and would be proud of a nursing license and you're taught to put your patients f

The staff at Four Oaks Healthcare are awesome!! It’s a very caring and nurturing environment, and everyone works together to give the best possible care for the residents.

Staff was extremely nice and helpful in giving me directions on how to get to my Mother in Law’s room. She seemed happy and well cared for when I arrived. Everyone was cheerful and dressed in holiday attire. Downside was it seemed terribly overcrowded. But, overall everything seemed to be running smoothly from what I could tell on my first visit.
