I am heartbroken and deeply frustrated by the severe neglect my father experienced over the past two years at Sage Point/Green Acres Nursing Home. He was a long-term facility resident, for many years, but the past two years, we noticed the decline. The lack of basic care and attention to his needs was appalling and caused his health to deteriorate drastically. Malnutrition: Despite multiple requests to improve his dietary plan, my father’s weight dropped significantly over time. He became severely malnourished, weighing only 100 pounds at his worst. It was clear that he was not receiving proper nutrition or assistance during meals, even though we raised these concerns over and over again.
