Ceres PostAcute Care

Please I beg, if a patient needs long term placement DO NOT send them to Ceres Post Acute. It is an awful place. I feel compelled to share our experience in the hope that it prevents another family from going through what we did. My father was transferred to this facility for post-acute care. Within a very short period of time, there was a noticeable and alarming decline in his condition compared to his status at the prior hospital. He went from being able to communicate, eat, and walk with assistance to suddenly struggling to breathe and unable to speak in full sentences. When concerns were raised with staff, communication was extremely difficult. We were unable to get clear or consistent information about his condition, medications, or care plan. It's been a Rollercoaster since july 1st. This place was great my ex was only their for 5 days but the staff is exceptional compared to the 2 other places hes been, and they finally got him transferred to Fresno, which is what we waned from day 1, thanks to ranjib, you guys are Great. We almost don't want to transfer him since they are great, but this way his family can see him more, Thank you all again
