Avon Place Skilled Nursing and Rehabilitation

A very dear family member was there for 2.5 days for rehab from a stroke. He was not treated well at all!!! Left lying flat for hours and wasn’t supposed to be lower than 45 degrees due to a feed tube because he couldn’t swallow due to the stroke, he wasn’t turned and resulted in the reddest bottom ever, nurse had auditory to say he came in with it! Which he did not l!!!! photo proof as his wife watched everything very carefully!!! He called asked her to get help he was in trouble, she arrived the nurse said his vitals are normal which they may have been but he was having issues breathing, seeing double etc, nurse was asked to call ambulance she stated they can’t come til tomorrow…. His wife had to call 911 got him to ER to find out he had a clot in the brain, and contracted Legionaries D

This place is awful! My grandmother was being abused there when she went in for rehab after getting very sick. They WILL NOT speak to you on the phone. They wouldn’t let us see her (granted it was during Covid) and they would leave her in her own feces and urine for HOURS and when she would ask them to turn her cause she was uncomfortable they they would throw a blanket at her and tell her they just flipped her and to figure it out. This is an absolute horrendous place! DO NOT do that to your loved ones.

PLEASE SAVE YOUR FAMILY MEMBER! My family member told us she had been hit by someone, we told the nurse, the nurse did nothing; said “that’s what she always says”, 1 day after telling us she had been hit by someone, she was rushed to the hospital and has 2 broken ribs. The facility is now telling the family that they can’t prove when she had broken ribs and it could’ve been any time within the past 2 weeks. We have no info about the investigation and everything is being denied. If she had broken ribs for 2 weeks and it went unnoticed???? What is going on here????

My mom is the happiest she has been in months being there. The care she has received is wonderful. Joni and Katie are amazing and the aides and nurses are caring and helpful. Very grateful!

Avon Place is absolutely amazing! Both my Dad & my Grandma stayed here & the staff is so caring, the rooms were spacious & the building was always spotless. They always had activities to do which made their time enjoyable. The food was always hot, fresh & homemade. Dietary manager Michelle was so helpful and kind & always helped my Grandma fill out her menu and made sure she got what she liked. I love that they could eat with other residents if they wanted and not just in their room. They even offered me guest trays at no charge. I couldn't be more happy with them.
