This review is directed to the Medical Director. I requested a formal internal investigation regarding my primary care physician’s management of multiple serious medical complaints. I was notified that the investigation concluded and determined my physician acted appropriately. At no point during this process was I contacted, interviewed, or asked to provide documentation. An investigation conducted without speaking to the patient is incomplete. The following are factual examples from my care: • I presented with persistent left arm pain. I just wanted to take a moment to say that the joint team that worked with my husband when he had to have a hip replacement they were so prepared so helpful to help me to know how to take care of things ahead of time that Jamie I believe the head of the team the nurse who put things together + the team they were absolutely amazing. Also the staff that worked with my husband when he got back after his hip replacement were all so kind and helpful . I can say that's the greatest quality of care that was so appreciated. Dr Dominick was his surgeon on July 25, 2024 . Thank you everyone for your help.

I have seen Dr Cole on two separate occasions and he’s very forthcoming and courteous. He suggested that my surgery would take place sometime in October. I attended my PT evaluation on September 15th only to find no surgery had been scheduled or a prior authorization was not submitted. I contacted the Nurse Navigator only to be told my insurance would probably not approve of my surgery unless I lost approximately 20 lbs. I contacted my insurance only to be told that was not true. After many messages and calls I have a surgery date for December 10, 2025. The staff at Dr Cole’s office is still demanding I see Weight Management/Nutrition to have a plan to lose weight. I have little mobility with knee in question and walking is painful, however I do the “exercises” suggested by PT. The office

I had one day surgery at Evan on Oct 7. I was shocked at the lack of care I received from the nurse at the last stage where I got dressed and released. The nurse after surgery told me I would get some crackers and a pain pill before I was released. However, the next nurse (releasing nurse) gave me no crackers or food, and did not give me my pain pill until I complained to her. She was mad I should stay longer if she gave me my pain pill but left me go right away anyway. Discharging nurse did not go over my release instructions. She just wanted me gone ASAP. Also, I had my handicap placard in with my clothing and it was GONE when I went to get dressed. Nurse claimed I must not have brought it but I did.
