The Citadel at Saint Anne Place

I am writing to express my concern regarding the substandard care my mother received at Saint Anne’s skilled nursing rehab facility in Rockford, Illinois. During her approximately 3.5-week stay, I made the decision to admit her due to a fall at home, despite her ability to stand, walk, and use the restroom independently at the time. However, she rapidly declined in care, and I witnessed multiple incidents, including a recorded conversation with a nurse refusing to provide proper care. Following these incidents, I traveled from Georgia to Illinois, demanded her discharge, and took her home to her apartment, where I cared for her for approximately 1.5 weeks before relocating her to Georgia due to her inability to live independently now . This place is a joke. My husband in there 3 weeks do one ever cleaned his room or attempt to wipe off he's try. They said he had covid or they thought. After 12 days he ask if he could be cleaned. Response if I have time. Never did. I have complain over and over. All I got we will get on it. I realized there are other patients. But this is terrible. My husband had a rash meas told they will be putting antibiotics powder only twice then stoped again we will get on it. My husband was bed bound per Dr. He called when he had bowel movement sometimes they would come then others eathier left him on comode. He has pooped in bed several times . A nurse aid could not stand the smell. She got another aid and looked at me in hall and shook her head... The food is horrible. They took my husband cup re

This was our first visit to our long-time friend who moved in the day before. Her room is spacious, nicely furnished, and aesthetically pleasing in decor. The staff is friendly and helpful, according to her daughter. The lift lounge chair is a huge plus. There is seating for our whole family, and the view outside is lovely. Her food looked appetizing, but she may not eat due to her mental condition. Her daughter was confident that adjusting the food for more likely consumption is doable.

As a long-term resident here, I find the care substandard. You can hit your call light and depending on the time of day it can take well over an hour for them to respond. There are some very good CNA’s here, but there are some very bad ones as well For example they leave your breakfast tray in your room and then don’t set you up to eat it. Right now my call light has been on for well over an hour and what I needed wouldn’t have taken but a few minutes, but it won’t be handled for another half an hour. I am in pain and this does not help at all. I remember when I had a friend here years ago, and the car was very good. It is no longer the same place.

This place shouldn't even get a star in my opinion. When I went to visit my family member last night and we walked in and her bed had a thin sheet on it and a very very thin throw blanket on it. No bedspread no thermal blanket just a little throw blanket. My family member gets extremely cold most of the time. And when I went to ask a cna for a bedspread she had a attitude about it and roughly hands it to me with saying u can put it on we are busy. Mind u at least 3 cnas are standing around on their phones. This completely unexpectable and I will not put up with this. They didn't even have the proper sling under her for them to use the hoyer lifting her. When she is discharged she will never be going there ever again.
