Slate Valley Center

What a beautiful facility. Everything was very clean and nice. The staff is incredibly kind and helpful. A special shout out to Mary Alice. She's the first person youll see and sets a warm and friendly tone for your visit. She's like your favorite aunt! Thank you to all the staff for taking such great care of my cousin. Its greatly appreciated. You do a great job. Thank you for helping us feel like we made the right decision. Very highly recommended!

Slate Valley rehab has a pleasant atmosphere, activities for patients, and nice employees. My only issue is they only have one van to transport patients to appointments and it is booked far in advance. My friend had to find a way to get to the hospital for a treatment.

I was involved in a motorcyle accident which occured August 25, 2025 and which left me with many injuries including a fractured pelvis . I chose Slate Valley because of its good reputation in rehabilitation. I was not disappointed. I was given sound advice and encouragement and felt that the entire staff was working as a team. I left Slate Valley much stronger and able to perform daily tasks that allowed me to become independent again.

Worse place every mom was abused my number of aids, they left her in her own mess for 24 hrs. She never got the rehab that she was supposed of received, she fell out of bed I was not notified til 24 hrs later. There is a lot of abuse in this place that goes unreported her roommate fell out of bed and nothing was done about it my mom was in there for 3 months at first she liked it then things started going on and when I was told what was going on they knew I was pissed the whole place heard me. This place doesn't give the rehab that people need. This place is a joke the day my mom left was a dam good day she is doing 100% better and she is walking with a walker when they told me she will need 24 hr home care the people that I had to come to her house said other wise so glad my mom is home

Everyone was very kind. The Christmas Dinner was a wonderful event that we appreciated very much. Thank you.
