Signature HealthCARE of Terre Haute

My experience with Signature Healthcare has been extremely disappointing. The level of care I witnessed is rushed, minimal, and far below what families should expect for their loved ones. Basic needs often seemed like an afterthought rather than a priority. The administration appears completely disconnected from what’s actually happening on the floor. They stay behind closed doors and are rarely visible or hands-on. When concerns are brought up, there’s little sense of urgency or accountability. On top of that, the facility itself is in poor condition. It feels outdated, worn down, and in need of serious maintenance and improvement. For a place responsible for caring for vulnerable people, the environment should feel safe, clean, and well-kept — and that has not been my experience here.

If I could give zero stars, I would. This place is the definition of neglect and mismanagement. The staff constantly ignore basic care standards, and it feels like rules are optional instead of mandatory. Residents are left waiting far too long for assistance, call lights go unanswered, and basic hygiene needs are clearly not being met. It’s obvious that procedures and regulations are not being followed. There’s little supervision, and leadership seems completely disconnected from what’s actually happening on the floor. My father is currently a resident at Signature HealthCARE. When I brought him back to the facility recently, we were unable to enter because we were not aloud to know the code for the door. There was no one at the front desk, and despite calling multiple times, no one answered the phone to let us in. This was very stressful for my father and I. A facility caring for vulnerable residents should always have a reliable way for families and patients to gain access, especially after hours. At the very least, there should be a clearly placed bell or alert system outside the entrance that notifies the nurses’ station when someone needs assistance. I hope Signature HealthCARE makes improvements to ensure residents and their families feel safe and supported when entering and exiting the building.

We had to place our family member in the facility in February for therapy. Since then, we have decided to keep her there long term. The staff has been amazing with her and very attentive.

I love being a patient here. I love the staff, they have become my friends. I love the food here and all of the people that I have met. Therapy helped me get two new prosthetic legs and I can stand and walk again. Tiffany has become my family and takes care of me. This is a great place to be and I am very blessed.
