I totally regret placing my husband in this Nursing Home. His name is Fred Scott Carlton, hust a little that happened.Before leaving him i explained in detail that he had to be encouraged to eat and help him eat, he had to be walked with assistance, they assured me he would be taken care of. He was to be staying 10 days. I carried him 7 sets of cloths and his name was in everything, i also left $120.00 for anything he needed, such as a hair cut which they said he would get which he never got. He was there about 3 days and I received a call he had fallen, but was ok. On October 12 th I received another phone call he had fallen again and had a big knot on the back of his head, said the doctor was checking him out. At that time I told them I would be picking him up the next day, but to call

I can't effectively convey how amazing and wonderful this place is. The entire staff took, not only impeccable care of my dad, but also of my family navigating one of the most stressful times in our lives. They CARE about every person there. They knew and remembered our names as we passed in the halls...from the Director of Nursing Lauren, to Jessica, to every nurse, aide and therapist (special shout out to Cynde who took the time to return a call from home on her day off while dealing with her own stuff for no other reason than she cares). They were lifesavers and took excellent care of us all.

I have been in this skilled nursing facility averaging 12 hours a day for over a week. Not one time have I heard a patient call and ask for help from their room. The patients seem very safe and familiar with the staff here. One thing that has impressed me is that there are patients that are close to the nurses station feeling that they're part of a team so to speak. The nurses know them personally, call them by name, and will chat a little bit about their personal lives. I am very, very impressed with this facility. I have done this kind of work for many, many years. This facility is an exception in a very positive way. I hope all this makes sense I'm 82 years old and I'm tired.

I had rehab at S of H for 5 weeks Aug-September of 2023 after breaking both legs. Not gonna lie...had a rough start. I was at the mercy of busy staff for bedpan needs for the first 5 days until the therapy team taught me to transfer myself to and from platform commode and wheelchair. Once I was as independent as possible i got to know and appreciate the staff and had tons of support from friends and family. Wouldn't want to go through it again but God taught me a lot during this experience.

My dad was in this facility on two occasions. First was to recover after a fall and hip replacement. He found the experience there appalling. Lack of care of him and others from his own testimony is enough said. He recovered enough to go home that time. The second time was 2024. He was at the end of his life, on hospice, and immobile. My mother had no choice but to take him back to this facility. She was there every day of his last three months of life. She was upset daily, that he would be left to urinate on himself, dirty sheets, no response to the call button, it goes on and on. The staff constantly communicating lack of workers and they couldn't keep up with the needs of patients. In the last days they were giving him high doses of opioids to ease him out of this life.
