Sagebrook Nursing and Rehabilitation

My father was at Sagebrook for a short stay of 21 days. The facility is very basic and older, but clean. Megan, the Nurse Practitioner and the nursing staff and PT team were very caring, gentle, positive, friendly and professional, always going out of their way to make sure my father was well taken care. They did a great job keeping us informed, through email, phone and 1:1 conversations. I recommend Sagebrook.

I was there from Dec 2 - 25th 2025 with 2 broken legs a wrist and collar bone and had therapy there with Jordan and Lee and they were great I wasn't able to walk or move my right arm and Jordan did a great job getting my legs working and Lee with my arm. The nursing staff were great very friendly and professional with a shout out to Evie she is a great nurse and I can't forget Jeff another awesome nurse. Thank you all for everything you did for me

This facility was deeply concerning to experience. A hell hole is more like it. During my visit to a family member, I observed a complete lack of meaningful activities, subpar cleanliness, and an overall environment that felt bleak and disengaged. Many residents appeared isolated, sitting silently with little to no interaction or stimulation. The experience was troubling enough that our family held an emergency discussion immediately afterward to consider relocating our loved one. It raises serious questions about whether there is an activities coordinator or structured programming in place to support residents’ emotional and mental well-being. Sagebrook may look like it's an older building, not as shiny new as some others, but you won't find better care anywhere! All of the staff treat my husband like he's family. From nurses to PT, 5 stars!!

Therapy was good even though none was done on her shoulder after the surgery. She felt there was no connection with her doctor or medical history. As to the question below about price, I believe it was all covered by insurance and I don't know what they were charged
