My mom was in this facility a little over a year ago and I have just been able to bring myself to write a review. The guilt I have felt for leaving my mom in this facility has taken its toll on me this past year. My mom had COPD, had a hospital stay and came to this facility for rehab. My mother lived with me, but became ill while visiting my sister, my sister lives near this facility. I live about 1 1/2 hours away. I came to visit my mom around 11:00 am one morning. She was still in her p.js, and a breakfast tray beside her that was uneaten. She had tried to make an attempt to drink her coffee and had spilled it all over the front of her p.js. As soon as I walked in she started crying and said she didn’t feel well. That her stomach hurt and nobody would help her! My husband and I sat

A family member is immobile with a broken hip, yet she was left in a soiled diaper and wet bed all night. Our family thinks this is blatant neglect. These issues have been reported to administration with no results. She is at high risk for sores and infection due to their lack of care. We will now be attempting to contact social services and other agencies too ,so far we've had no reply. I am now posting this picture to entail the true conditions that we've witnessed here. I will be back this week and will update this post. as this seems to be an ongoing problem. What did these people do at night? because this was early in the morning.. it won’t let me post this picture

Would give them negative stars if possible!!!This place has no communication with family members. Once she got there they called and left a message of date and time for meeting without asking if time was good for me. I did call back and left a message that that time would not work and no one called me back, although it was rescheduled for a later time for same day. Luckily I was there and was told we will be coming to get you shortly for meeting. Was told she would be there 20 days and went over all medications and they still couldn’t get her medications correct. Told her she had to leave after less than 2 weeks but did not tell any family members. Tried to discharge her on a Sunday and she lives in assisted living and cannot go back on weekend and they also never sent orders for discharge

The communication between nursing, tech, pt's family and pt is HORRIBLE!! Staff does not answer pt calls promptly. We had to request multiple times for medications. Wound dressing was not changed routinely. Antibiotic for UTI was not given and reason given is "system flaw" and it was canceled. UTI culture was a bacteria that is contracted person-to-person. I did not once see staff sanitize any medical equipment when taking vitals or changing wound dressing. I asked my dad's current weight and was told a 100lbs difference from admitting weight. The nursing staff not once spoke with us about dad's declining appetite or weakness. We had to approach them. The only area that is worth mentioning in a positive light is Carlos in physical therapy. This is our 2nd experience with The Oaks at Limestone. My mom is there now after breaking her hip and my father was there exactly 2 years ago after a stroke. What I find most impressive is finding the same staff members, PTs, nurses, etc. that speaks volumes in the healthcare industry! Everyone has been so pleasant and very helpful just like last time. The place is very clean. I am always surprised to read some of the negative reviews. I would recommend this facility every time!
