Patricia is knowledgeable and took time to explain the care that I received as well as the benefits for changes to improve my at home routine! One thing that surprises me, is that when moving from one FORWARD location to a new one, my records are NOT accessible to the new location. I want my new provider to have my dental history. How can I ensure that they have access?

What can I say? This practice is very well run. The people are friendly, I was greeted by name and I was seen on time. And most importantly my cleaning was painless. I highly recommend this dental practice.

Excellent service in having my periodical cleaning completed by the dental hygienist Betty. She was very thorough and personable throughout the cleaning process. I highly recommend Forward Dental Mequon.

My husband and I are extremely disappointed with the front desk service at the Mequon location. My husband got charged over $200 for cavities that did not exist. We called and they said they would refund us and we have yet to see that refund. Additionally, I have sent no less than 4 emails to them requesting assistance with another $80 charge that came out of no where as well as some paperwork and it has been radio silence. Terrible service and charging for nonexistent services.
