Forum Parkway Health and Rehabilitation

I attended a Thanksgiving lunch for families. The staff amazed me at their attentiveness! I saw residence struggling to reach their food and just as quickly, a cna or other staff member was helping them. The activity director LadyBug was full of energy and made a point to interact with the families. The residences seemed to smile and enjoy interacting with every staff member they encountered and gave me a very good feeling about the whole facility!

My time as a long term resident has been an easy transition so far. The staff here at the facility have made my life easier and more eventful. The activities director has even implemented me into having my own wheelchair workout class in the morning. The receptionist Ashlynn and Mitchell are exceptional and extremely kind. I would recommend Forum Parkway for your loved ones.

The level of quality / dignity service has declined drastically in this location. We had My 93 year old mother stay here twice. Once for rehab and another for respite care for 7 days. I could not believe that the level of service had changed so drastically. One could say they were either short staffed or just stopped providing the same quality they use to. I saw plenty of staff just not the caring side this time around. On top of all that, we are still waiting on a refund that the administrator told me was is in the mail. That’s going on 3 months now. We had to get hospice involved as they had paid as well. This tells me nothing is urgent from the staff to the administration of this facility. Totally different experience from our first time. The staff was so caring and very helpful The front desk to the gym everyone was outstanding. If I needed anything the nurses or anyone that would pass through was very helping. I recommend any and everyone to try forum Parkway rehabilitation facility

The staff was amazing. Came for rehab but ended up having to have hospice come in and graduate my family member on to heaven. Team worked great together and we had the best experience ever here!! High recommend
