Let me first say that the experience we had there with my dad was nothing short of exceptional. Could there be some areas of improvement? Absolutely, every place falls in that category. But, from the nurses, aids and staff, they were wonderful. I have to give a big shout out to Cheryl and the physical therapists. My dad could barely move his arms and could not move his legs at all when he got there. Thirty days later, he walked out of there! Last, but not at all least, is Bre. She might be the most wonderful human being you could come in contact with. She was right on top of all our needs and requests. She truly is a caring soul. Like I said, no place is perfect and to believe that a place is, is naive. However, Five Points exceeded our expectations!

This care facilities (owners/investors) suffers from the same things as all the care facilities I've dealt with---understaffed, poorly paid staff, promised care they have difficulty providing yet collect money for services. My family member has gone up to 4 days without baths. When you call to request help, you get referrals to "nursing" where no one answers. When you do get to talk to someone, you are told "They can get a bath whenever requested." Yet they don't get one. God help us. Maybe our only hope in this world is for AI (robots) to drive all these facilities out of business.

The facility is immaculate, staff is friendly and caring. They made the whole process of admitting my mother effortless. I highly recommend 5 points over any other Nursing Home in the area. Mom really enjoyed the chili cheese dog, tots and sourcrout for her first meal there!

Let me say first of all that there is no perfect nursing or rehab facility, but in my opinion Five Points comes as close to that goal as one could expect. Selecting a rehabilitation facility is a daunting task for which few of us are prepared, and after looking at several others, I decided on Five Points. Krysta and her team run a clean, orderly, and welcoming facility where the staff are friendly, conscientious, and competent. The four halls converge on a large, bright, and open area which contains the nurses station, offices, and public visiting space where there are pleasantly arranged tables, chairs and sofas. There is even a faux fireplace that generates heat, and artwork on the walls. A very pleasant space indeed. My 80 year old friend for whom I am caregiver, has just been discha

I have been a resident at Five Points since summer of this year (2024), having moved from Fortress NH after suffering a major seizure in 2021. Adjustment to life in a nursing home is a jarring experience for virtually everyone, as one is uprooted from their home & family and must acquaint/adapt to new family here. I’ve seen many people imploring to understand where they are, why they’re here, or have been abandoned (as it seems to them). Been there, done that. It takes time to acclimate. Families, of course, suffer also from loss and confusion. Skilled care is a loving choice for loved ones who need it in a great many circumstances. Five Points Nursing & Rehab is a shining example of how to do long term care right, because caregiving is about people. Being a new facility (opened in March
