EverGreen Commons Rehabilitation and Nursing Center

Although they are chronically understaffed the aides do a good job & seem to really care about the residents. Unfortunately they have nearly everyone in a wheelchair for mobility & decreased fall risk, they stop walking residents that need assistance. Sometimes the wait to use the bathroom has been 1 1/2 hours. Many complaints about the food (chicken,pork & mixed fruit cup) being terribly repetitive & many times cold. Oversized mattress (air mattress) does not seem to have correct size fitted sheets available. Many different reasons have been given for these issues. After being a nurse for many many years, I do sort of understand some of these problems. They do an excellent job keeping the building clean.

Set my mom there about a month ago for rehabilitation and they have done a great job with her. facilities are clean ,staff is friendly and my mom really seems to enjoy being there. She is almost ready to make the transition to long-term care and she seems to like it, so I think this will be where she will stay.

My husband was a short-term rehabilitation patient in September, 2025. The care, attention and therapy he received was excellent. Every person on the staff was welcoming, efficient, respectful, friendly and capable. Both my husband and I highly recommend Evergreen Commons!

I was very pleased with my visit to a dear friend who is in the rehabilitation program at Evergreen Commons. The grounds are well cared for and the building is neat and clean and welcoming as I entered for a visit. Receptionist staff we’re busy talking with other people so I had a bit of a way to check in, however as I found my way down to my friends room: I found the halls to be decorated with attractive and contemporary artwork and encouraging Plex. The activities room is bright, colorful and stocked with puzzles and activities resources for residence. Residence rooms are spacious and sunny and have a window that looks out on a green area. My friend and I attended an outdoor cookout with musical entertainment activity for residence and guest. Food was good and well prepared. I would give

Visited my Aunt, who was celebrating her 95th birthday. Staff were very helpful in directing me to the lounge where the family was meeting. The atmosphere at Evergreen seemed upbeat and positive as I made my way to the lounge. My aunt and her family are very happy with the care she is receiving.
