Eugene Tanquilut, DO, FACOS

I had seen Dr. Tanquilut for needing shunt or graft for dialysis. I was very impressed with the doctor he was excellent in explaining the procedure to myself and my family. Unfortunately from there it went downhill fast. I was told surgery was September 14 th, and I would receive a phone call prior to surgery with instructions and time to be there. I tried to get in touch with the office once labs were done to confirm they had the labs. I also needed time to be there as family was coming in from Indianapolis to be with me. No one ever called me back. I finally reached a staff person who said oh your surgery is in Munster and address was given to me.I continued to wait for the pre op call. NO CALLS to give info I needed.went to location given to me no one there it appeared to be closed. I
