You're Fired! Went last year and later requested they fill out a simple one page form for Lasik Plus. I even took the form by there and handed it to them. "No problem", they said. Never got it. Went back a few weeks ago and requested the form be filled out. Took them another copy. "No problem", they said. A very kind and diligent assistant there did her absolute best to get the form filled out for me. She tried for several weeks. She called me today and told me that they said I'd have to come in for another in-person visit to get the form filled out. So I am done with them. It would literally take them 3 minutes or less to fill and sign the form, but I guess that's just too much to ask, given their whole focus on production quotas and making sure they hit their sales marks on contacts,
