Would give them a 10 star but only 5 were available! They have been amazing taking care of our 93 yr old mother as if she was their mother. Nursing her through a tough battle of pneumonia recently, all staff, nurses, CNAs, therapy staff, cleaning staff and others have been so thoughtful and caring and friendly to our Mom. Facility is very clean as they are constantly working to keep it that way. I would highly recommend them for your person needing constant care and attention. We have no regrets about placing Mom in the hands of the staff at Azalea Health & Rehab! Elaine Odom

Our mother was in this facility for over 2 years but unfortunately lost her battle with dementia. The care she received from the entire staff was incredible. They always kept us in the loop about everything going on. I give every department a 5 star rating.. It was a blessing for her to have been cared for by this facility. We will forever be grateful and blessed to call them our family..

My mom was here for rehabilitation after a bad fall and they took excellent care of her. From the business office with Jennifer to the nurses and wonderful physical therapists! They even let me bring her little dog to visit on the porch. They were all professional yet friendly and caring! Susan, the activities director, even painted my mom’s nails. She was well cared for and I would recommend them to anyone. Thank you all! Barbara Griffin

Our family is thankful and appreciative of the care that the staff at Azalea provides our mother as well as our father before he passed. They care about the residents and try to make each resident as comfortable and happy as possible. Anything we’ve ever asked for has been done! We appreciate all they do!

Great staff! I appreciate all of the love that mama received especially from Tiffany and Kim. Jennifer was great to work with on the financial side and Susan always had an activity for the residents.
