Our family is deeply grateful for the care our sister Cynthia received at this nursing home. From the moment she arrived, the staff treated her with kindness, patience, and genuine compassion. During a very difficult time for our family, it meant so much to know she was in such capable and caring hands. We especially want to recognize Nurse Syronda Basset. She went above and beyond in every way. Her communication with our family was excellent—she always kept us informed, answered our questions, and made sure we understood how our sister was doing. More importantly, she treated our sister with dignity, warmth, and true attentiveness. I would like to share that this facility is excellent ! My Aunt Is currently in here for rehab and I would like to thank Jackie for making my Aunt feel so welcome and going to the store to buy her anything to make her stay more Pleasant. My Aunt also loves Aide Tracy and the evening nurse Gene. I would highly recommend this facility. Especially when my Aunt Is very hard to please. They also give you a Voucher for a free haircut and blowout. Thank you for making my 85-year-old Aunt look beautiful. And getting her strong at the same time so she could go back into her home instead of a nursing home.

Just want to note that Jackie Costa and the kitchen staff were excellent with the service they provided during my stay. The variety and quality of food was very good and consistent. They showed a lot of flexibility and were always very willing to provide me with substitutes even at the last minute. Keep up the great teamwork!

Autumn Lake is terrible! We had my father here for a week after he suffered 2 strokes. He needed subacute rehab for a month and we could tell almost immediately that Autumn Lake was not going to be a good experience. Rooms were dirty, smelt like Urine, flys everywhere, call button either didn’t work or worked and took forever to receive care. Overnight staff was even worse. They dislocated my dad’s gall bladder drain .. didn’t call us .. we learned by just coming in and noticing it. Blamed him! Totally incompetent facility. We ended up transferring him to Florham Park Rehabilitation Center and it was wonderful!!! He had 2 nurses at Autumn Lake that were lovely - Nazheria and Lorna. These two beautiful souls are the only thing autumn lake has going for them.

My husband went to Autumn Lake in Berkeley Heights on a Saturday around 2;40 pm for physical therapy on his legs to get them stronger. Sunday morning he was found dead on the floor. When one of my daughters got to Autumn Lake on Sunday morning to spend the day with her father, Rhonda my husband’s RN told my daughter her father was trouble all night. She said he kept hollering in pain, falling out of the bed and she had to keep picking him up. Then she told my daughter a man told her my husband was unresponsive and Rhonda had to do CPR on him and she’s all sweaty from that. My daughter ran out she couldn’t take anymore. A few minutes later my other daughter and I arrived at Autumn Lake. Another nurse followed us into my husband’s room. That nurse told my daughter that was with me they fou
