My giving this a two-star had absolutely nothing to do with the care that I received because it was exemplary. Staff was attentive, physical therapy was amazing. They have a very active activity department. The food is really good as well and they have a Deli menu where you can order something different, like chicken tenders or sandwiches, if you don't like what's on the menu . My low rating was because I feel like I was pushed out after 30 days. I wasn't any better after 30 days then I was when I went in. Also, their Social Services Department is so bad. My discharge paperwork said that they had contacted Ascension at home for my aftercare physical therapy. A month later they still haven't received an order. I was there in last September. The worst palace to be sent to. I would love to see this place shut down . Pt/ot was amazing 10-10 just a blessing. The staff is another story no one ever knew anything when asked they were all dismissive and had a chip on their shoulder! They were defensive and argumentative 100% of the time. Easily agitated when you hit the call light which can be up to 6 hours before they respond to you. And the attitude is the if you interrupted their day . If you have as need meds you’ll get them when they feel like it cos you’ll be reminded they are as need so they aren’t scheduled so they don’t have to give them to you at all. I watched several people die there . I was discharged without papers or them even knowing I was discharged they didn’t follow through with my

My mother has had trouble in this place constantly since being in this facility, from nurses not fully knowing what they’re doing, to not doing their job, and some tech’s being extremely rude, and more. Wouldn’t be my first choice to send a loved one.

Please don't let your loved one go to this place. I could go on and on about how they were giving my mother's meds 4-6 hrs past time, the food is upsetting to watch people to have to eat. Didn't have milk or juice for a week or so in the beginning of my mother's stay. Rude people working there. My mother had three packs of cigarettes come up missing and the staff said there was nothing they could do about it. There are a few nice people working there but that place absolutely needs to be investigated by the state.

Staff frequently leave patients in chairs in the hallway all day and most of the night despite them crying and wanting to get into bed. Patients are told they get a shower 3 days a week, but its usually once or less a week. Instead of being assisted in the shower patients are frequently left alone in the shower in a chair and have to wait on someone to eventually return or try to get out themselves. The food is atrocious and very little portions. Commonly it would be a small serving of veggies and a dessert. Breakfast was typically half a piece of toast and a bowl of rice...and ALWAYS cold. Staff vs staff and staff vs patient altercations happen frequently. HIPAA doesnt exist there. There were very few staff that were great and actually cared for their patients.
