This facility is supposed to be a non-smoking nursing/rehab facility, yet you have residents sitting right outside the main entrance smoking. This was observed today, 9-14-24 at 3:45 pm and 9-12-24 at 11:50 pm. This was brought to the attention of the staff today. There are other issues at this facility that will be addressed in a different manner. That being said, I would not recommend bringing your loved one to this facilty, name changed to The Reserves. I have photos but will not post at this time due to not being able to blur residents' faces.

I would not send my father back here again. The care was awful. He was there over night and we had him moved. Trident hospital sent him there and should not have. He was still sick. They let my dad lay there and did nothing. The following day the employees told my mother people were coming in all day to do things and no one ever came in to check on him. They did not help him with his breakfast. They just took it away without trying to feed him. Family was not there yet that morning. By the afternoon he was not responding. I feel that he would have died if we would not have had him taken to a hospital that day. It definitely was not Trident. We took him to a downtown hospital. He had a long road ahead of him. I am very upset and disappointed in this facility. My mother was transferred there on Monday and I wasn't able to talk to anyone, including her for 36 hours, despite leaving multiple messages. I have decided to have her transferred to another facility and today I have not been able to reach anyone there by phone, it goes straight to voice mail. I feel like Viviant is holding my mother hostage to get that money for extra time. If I could give less than 1 star, I would.

Horrible EVERYTHING. First of when you walk in there's an older lady sometimes and she's so rude. I left something in my car two times and went back two different times. When I came back the second time the receptionist with waiting for me with a horrible attitude telling me I had to stay in or go out. I do not need her permission to move my body where I need to go so that was aggravating and rude. The Second time in one week I had bad attitude service was when I was in the Azaela Cafe doing some work while my grandmother had visitors. The area is always nasty and sticky because they never take the time to clean it. I'm doing work and a nurse comes in and rudely ask me if I'm waiting on someone and why I'm in there. My father-in-law was just discharged yesterday. This place is horrible! I don't even know where to start. On the day of his admission, we couldn't get any information about the facility, a care plan, what we should bring him or what we couldn't bring for him. Everyone we asked said they would be right with us. After 2 hours and no one helping us, we gave up. EVERY time I went to see him he was in bed, hanging half off. His call light was always out of his reach! He fell several times while he was there. Once we knew he was going to be discharged we had a meeting with PT, the social worker and head nurse. They were all very nice and told us they had ordered what he would need at home. When we went to get him we found him sitting in a chair in the hallway holding a bag. We went to his room t
