TRI-STATE REHABILITATION and NURSING is the BEST facility mostly due to the EXCELLENT Administrator doing great job! I had experience searching for the right place for Mom and Dad. Tri-State can't be beat. Thanks again for looking after them. Sincerely, Melinda Lau Recently I have read numerous complaints about various nursing/rehabilitation facilities regarding upkeep of buildings and cleaning, (eg shower areas, floors, etc). Machines not working, (eg clothes dryers). Some residents not taken care of properly and other problems. TRI-STATE still great!

I went to the pysility to visit my brother who is still currently there. If you care about your loved ones DO NOT SEND THEM TO TRI STATE!! Please. When I arrived I spoke to my brother he stated he hadn't had a bath in a week. One time he had a wet bed he turned on his light , he layed there for over an hour, when I arrived I went to the nurses station it took him additional 20 minutes to get there to help change him and his bed. He was supposed to have restorative therapy and they said he would have it 3 to 5 times a day. We saw them once that week, she came a second time and said she would be back. We didn't see her again, case manager was rude. We talked to the nutritionist and he was supposed to have a nutritional shake with lunch and dinner. He had 2 in a week and didn't get anymore, t

We finally got someone I know OUT of Tri State to a MUCH BETTER facility recently....for them to put him in quarantine because HE WAS COVERED IN BED BUG BITES AFTER LEAVING THE TRI STATE FACILITY!!!! This happened within minutes of him arriving at the new facility. The new place (NOT TRI STATE) has every new patient receive a physical examination. DO NOT SEND YOUR LOVED ONES THERE!! The service is sub par. No one wants to do their job. Whoever is in charge NEVER listens to the families. Any help you get is because you have to constantly remind them, just fo them to argue with you. EVERYTHING....and I mean everything...is an argument. This bed bug crap was the final straw for me. BOTTOM LINE....IF THEY WERE DOING THEIR JOB, THEN THERE WOULDNT BE BED BUG BITES ON ANYONE LEAVING THE FACILIT

I'm an RN and resident of mine received acute rehabilitation at this facility. I corresponded mostly with Dominique in Social Services over the phone and eventually in person. Her assistence and follow through was fantastic- including added follow up to make sure i received important documents for review. The atmosphere upon my visit to the facilty was friendly. Staff were kind, helpful and knowledgeable about my resident. Overall, im happy with the care she received. I will be keeping this facility in mind as a top preference - and I look forward to any future correspondence with Dominque as well!

My mother was put in this facility from a referral for Hospice Care. Understanding no facility is perfect! But the care my mother has received so far has been great. Lynneice the DON very friendly and welcoming. Always responds to my calls or texts in regard to my Mother's care. If any issue does arise, she is on top of it. My mother has been doing well and states she likes the facility. I would highly recommend Tri State if you had to put a family member in a Nursing Facility.
