had an apt here for a cysto and when I was given the Patient Consent Form to review and sign, I was already in the procedure room. The nurse gave me the form, starting with page 3 and said to sign. When I asked a question or 2, she could not answer and said, wait here - and went to get the doctor. When I asked the doc some questions and what this or that was and why the form was blank ( no diagnosis , etc. filled in etc....) , he said to take the form home and read it and let them know if I had any questions. They then cancelled my apt.... Maybe they should give the form out in the waiting room, starting with page 1 and have all the blanks filled in!
