Overall I thought in all avenues and areas of care that the 3rd and 6th floor is phenomenal. Great nurses and aids and Phoebe the social worker was amazing. Her ability to follow up was beyond excellent and everyone was 10 steps ahead. It's a great thing when your loved one is in a facility that you don't have a worry or a concern. The maintenance crew were always on point. I'm so grateful because when she came in she was in a grave state and she was not at her baseline. The staff worked with her and she is now improved tremendously and is on the 6th floor. Kimberley from Link is phenomenal and makes sure everything is taken care of. I cannot say enough wonderful things about this nursing center and rehabilitation facility. The staff makes you feel welcomed, supported, and truly cared for. The nurses and therapists are not only highly skilled and professional, but also compassionate and attentive to every detail of care. The rehabilitation team goes above and beyond to create personalized therapy plans that truly make a difference. The facility itself is clean, comfortable, and well-maintained, which gave us great peace of mind. Communication with family members is clear and consistent, and any questions or concerns are addressed promptly. My 88 year old grandmother had a wonderful stay and well taken care of when she stayed at the Phoenix for 2 months (Nov to Dec 2025). The facility care team were very friendly and knowledgeable. The discharge process went smoothly with the help Cathy and Brenda was a great on helping to extend her stay due to insurance issues. I would definitely will send her there again in the future if needed. Thank you the Phoenix for all the support and caring for my grandmother.

I am extremely disappointed with the care my mother received at this facility. During her stay, her overall condition declined instead of improving, which was very alarming for our family. There were multiple instances where her hygiene was neglected — she was not showered regularly, and basic personal care did not meet the standards we expected. We were also concerned about medication management. We felt that her prescriptions and dosages were not handled with the level of attention and accuracy that a patient in her condition required. Another major concern was the lack of consistent on-site physician oversight. We were uncomfortable with certain treatments being administered without what we understood to be proper consultation with her primary doctor or surgeon. Our experience left u

I genuinely had a good and favorable experience at Phoenix Rehabilitation. The service and staff are a cut above the rest. They listen to their residents and show attentive, caring help and social support. Visitors are welcome and accommodated in caring for their lived ones. Consistent presence and follow through from doctors and staff in Leadership, Concierge Orin and nurses, personal care staff, wonderful Physical and occupational therapy very professional, knowledgeable and encouraging.
