St. Bernard Nursing & Rehab

The facility itself was acceptable. It was clean and there were no bad odors. The care my mom got while there for skilled nursing was good. However, the social worker there needs to be replaced ASAP - We tried for over a week to get her to send referrals to several nursing facilities for long term care. We could not get her to return any calls to us. We left several messages for days. On the 20th day (the day she is supposed to be discharged, or we have to start paying for it) we called and left 4 messages trying to get her to help and fax a referral to a facility - she would not return calls. We asked for a supervisor and was told she did not have one that she was given all of our messages, and she will call back when she can. My mother came in fine and was actually progressing after a devastating stroke, and left with severe dehydration and sepsis. Protect your people and bring them home

Provided horrible care. They didnt take care of my mom at all and they didnt respond to the nurse call button. Do not send your family here. Worst bedside manners ive ever seen. I hope to see this place shut down.

This place is the house of horrors. I visited a friend's dad there and this would be the last place I would send am elderly person in need of care. The staff have attitudes, when you ask questions and the rooms and bathrooms smell like the sewer. The staff treat the patients like prisoners and it's a sad situation. There's a GREAT NEED FOR A NEW STAFF, THAT CARES AND NOT JUST LOOKING FOR A PAYCHECK!!!! Management and or the General Manager should observe what's going on in this facility before something happens that they can't come back from.

My cousin was there a little over 24hours and it was the WORST EXPERIENCE EVER ‼️She came for Rehab after being in the hospital for over 3 weeks 2 weeks in NEURO ICU after having 2 surgeries.She arrived there approximately 5pm on Friday.I came earlier to check out her room and it was DIRTY But Admissions assured me it would be cleaned.I returned approximately 8am on Saturday and she was in the same hospital gown from the day before,no rail on the bed,wall where the bed is was Filthy with unidentifiable stuff,the closet and drawers were dirty,the room and bathroom was Stink and Reeked of Urine‼️‼️‼️I bought my own Cleaning Supplies and Scrubbed and Sanitized the entire room ‼️The Nurse didn’t have the correct information on her.She didn’t know she was confused,pulling and reaching, and extr
