If I could give Dr. Forman zero stars I would, unfortunately the score stats a 1. What he did, not tried to do. He knowingly and intentionally over charged me on my co-payment. I have a $15 copay. He instructed his staff tell me my copay is $40. I said no it isn't, I always pay $15 no matter where I go for medical reasons. Here's a little background. I was told I have to show him my private documents from EOB from my health insurance holder. Those are private documents and not for him to view if I don't want him to. But he insisted on seeing them and I kept telling him no! Keep in mind Dr Forman is in my plan he must Obeid by the plan's rules/amount of copayment. Or else he is in violation of the terms of the contract with the insurance company he himself agreed to when joining into the pl

Dr. Forman is the best - if I could give him 10 stars, I would. After several rounds of PT by the age of 22, I was given 2 options for my back problems as I could barely walk: Try a chiropractor, or schedule surgery. I went to Dr. Forman, and with his care I returned to not only walking, but playing sports. 15 years later, I continue to see him for ongoing support and maintenance. He is thorough, intelligent, patient, kind, empathetic, funny, and an all around good person. I highly recommend him, and am forever grateful for his care.

Dr. Forman is amazing. He truly cares about his patients and wants nothing more than to help them get better. He really makes you feel comfortable throughout the entire visit.
