Reliance Medical Group (Family Medicine)

I need medical information and vaccine records for my children who were patients in the Northfield office until it closed down. Ive tried to call multiple numbers and locations but there was just a busy signal. Please, if someone knows how I can obtain a vaccine record from this office, thank you in advance!!

Very slow sat in office for a hour then received a phone call while in the office asking me about my scheduled appointment. Ladies are not very friendly no eye contact no smile no friendly demeanor. Not very happy bout the visit Will be looking for a new place

Is like Reliance have a criteria to employ the worse from atlantic county. Rude people on the phone and at the office. But this one in pleasanvile has the number one spot. The place is not clean, staffs don't care about you,go anywhere else but this one.

I arrived for my 2pm appointment early, did not leave until 4:40pm for only a wellness appointment. When I arrived I asked if I should reschedule and was told I would be seen right away.
