Primary Care Plus

I followed a physician here that I had used for many years. I became so disabled that I will need in home care and have to be moved.I need a form filled out for HAA. I am enrolled in La Rapid Rehousing and am currently in a FEMA trailer. I have to have a form filled out to have my caregiver and service 🐕‍🦺 be there. They say I can't get it. An update, situation has been remedied. I have been in contact with Dr "Surrat" (I may have a I may have typed that wrong but that is the phonetic sound.) who is the medical director of the clinic. She has indicated, that while accidents or mistakes have been made, they are currently being reviewed and corrected. Paperwork has been completed and my situation has been remedied.
