My grandmother's fall was entirely due to negligence, as the outdated beds only have one rail. The aid refused to discuss her fall with me because I am not her power of attorney, but the nurse practitioner later asked my sister and me the same questions we've been answering for the past two weeks. Since there are no cameras in the patient’s room, we cannot determine if she hit her head. My Nana cannot advocate for herself, so even though she claims she’s not in pain, we won’t know her condition until much later. Twice now, she has been left with her door closed and curtains drawn in a room set at 80°, which only increases her confusion. The facility is severely understaffed following her fall. We had my significant other's father sent here as the distance from our residence was convenient for us. That in itself was a big mistake. When we came there to bring him to a doctor's appointment; we caught him having a stroke. He was sent to the emergency room and he was diagnosed with pneumonia and a high fever. He is a diabetic and half of his foot had to be amputated from surgery. Meanwhile, New Paltz Center fed him sauerkraut with sugar in it. His other foot had a huge blister on it that was caused by the diabetes! While he was sent to the hospital, they discharged him without the hospital's knowledge! If you have a loved one, please do not send them here. If you do, you might as well send them to the grave.

Tabitha, is unprofessional lacks communication and has put me on hold for over an hour and avoids having basic conversations… I would not recommend her for anything. She has lied, blamed others and never taken accountability for her actions. I would not recommend anywhere were she works

Visited a friend who is being treated at your great facility. Very pleasant staff, and everything my eyes had seen was very well main-tained.

Facility: C- Old and in desperate need of renovation. The facility is broken down to the point that anywhere you look you’ll see something that needs to be fixed or replaced. Maintenance tries to keep up with the most urgent repairs, but it’s a fools errand. It’s also in need of a deep clean. The outside area also looks broken down. All you see are dead shrubbery and dirt. Staff: B- This is a real mixed bag. Like many facilities, they are chronically understaffed. That’s inflamed by habitual calling out at the last minute. Some aides and nurses are very good, others not so much. Relations between the staff is internecine, with constant bickering and undermining of each other. At the root of this is that many of the aides are young and unprofessional. Many have commented that
