My father was taken to Levindale at 11 a.m. from Sinai Hospital. The doctor reviewed my dad's file and asked whether we would honor his MOLST. I said yes, and I never saw her again until she pronounced his death at 5 p.m. In addition, she listed that he had dementia, when he was as sharp as a tack up to his death. Levindale should lose its license.
