I can’t say enough good things about Liberty Center for Rehabilitation and Nursing. From the moment we walked in, the staff was welcoming, professional, and genuinely compassionate. The nurses and aides went above and beyond to make sure my loved one was comfortable, well cared for, and treated with dignity and respect at all times. The rehabilitation team was outstanding — knowledgeable, encouraging, and truly invested in progress and recovery. Communication with the staff and administration was clear and responsive, which gave our family real peace of mind. The facility itself was clean, well-maintained, and had a calm, positive atmosphere. It’s clear that the people here care deeply about their residents, not just as patients but as individuals. I’m extremely grateful for the care pro

Caring for an elderly parent can be overwhelming. The stress alone can take over your entire life, especially after a major surgery. When my mom had her knee replacement, we were anxious about the next step in her recovery. Choosing Liberty Center for Rehabilitation turned out to be one of the best decisions we made. From the very first day we arrived, the staff treated my mom as if she were their own mother. The level of compassion, patience, and genuine care they showed made an incredibly difficult time so much easier for our family. The facility was consistently clean and well maintained. You could see that cleanliness and organization were clearly a priority. And shockingly for a skilled nursing facility, the food was actually delicious — something we definitely did not expect! Over

Liberty Center is a horrible and deplorable place for any Human being to reside in! I had went to visit my Brother who was admitted in less than 24 hrs and will be leaving ASAP!! Soon as you walk in the front entrance, it reeks of urine. Two elderly patients were left unattended in their wheelchairs in the front lobby. No security guard was at the front desk. When a Nurse did arrive she asked us who were we visiting. Once we gave her my Brother’s name she escorted us to his room. Which concerned me. Because we were not screened at all. This a safety measure which needs to be addressed. Therefore Patients are not safe in there. The room my Brother is in is like a matchbox, small closet sized bathroom.. which is not Wheelchair accessible. Flies were swarming around in the room and hallway.

This place is not fit for living beings. It is in a state of ill-repair. It is filthy. My loved one complained of bug sightings throughout the facility. The entire place is eerie and does not deserve to be licensed. It is sad that this is the care that vulnerable populations are reduced to. Interesting enough, they show no pics of the patient rooms. ... because they are awful!!!
