Lake Pleasant Post Acute Rehabilitation

My father in law had an incredible time here. It was the first time a place actually felt clean, well kept, and the staff exceeded our needs. The place was spotless and the newly renovated rooms made it super nice. Couldn’t have asked for a better experience, and would recommend this to all of my loved ones

Our family had a very concerning experience with a loved one’s stay at Lake Pleasant Post Acute Rehabilitation (Jan–Feb 2026). Three issues stood out: 1. Condition on arrival during visits: On multiple visits, we arrived to find our loved one not positioned comfortably or safely and without basic items consistently within reach (call light, phone, and water). We often had to ask staff for help immediately. 2. Communication with family/MPOA: Changes in condition, care needs, and the care plan were not communicated consistently to the family or the legal decision-maker (MPOA). We repeatedly had to initiate follow-ups to understand what was happening. 3. Records and flow sheets (repositioning/hydration): When we requested documentation such as repositioning and hydration flow sheets/logs, we

My husband was transferred from the hospital to Lake Pleasant. He was there got twenty days or more for rehabilitation and that went well. The facility is clean and no bad odors were smelled. All his medication was given as ordered. The entire staff was very professional and polite. The nurses and assistants took very good care of my husband’s needs and wants. Lake Pleasant is a great place to be.

My father-in-law recently stayed at Lake pleasant Post Acute after a fall with broken hip at home for rehabilitation, and we couldn't be more grateful. The staff culture is truly what sets them apart; the nurses and aides took the time to greet us by name every time we visited. The facility was consistently clean and smelled fresh, which gave us a peace of mind. More importantly, the communication was excellent—we were kept up to date on all medical updates and therapy progress. Dr.Nagle the attending doctor communicated personally with us frequently. The activities and engagement with my father-in-law ensured us that he felt like part of a community rather than just a patient. I want to share my gratitude for the incredible care my loved one has received at Lake Pleasant Nursing Home. The wound director, Taryn, is truly a phenomenal nurse. She is exceptionally knowledgeable, deeply passionate about wound care, and fully dedicated to her patients’ healing. Her expertise and commitment bring comfort and confidence to both patients and families. I also want to recognize Becky and Erin, two compassionate and attentive nurses who consistently go above and beyond. They care for my loved one as if they were caring for their own family. Their kindness, patience, and genuine concern make all the difference in daily care. The team at Lake Pleasant has exceeded all expectations, and I am grateful every day for the outstanding care they provide.
