Grundy Care Center

Don't have personal experience...but based on reports from Feb 2026 this facility was placed on the Special-Focus Facilities list with Centers for Medicare and Medicaid Services. In Nov 2024 they were cited for 12 violations including failure to meet professional standards, inadequate quality of care, insufficient nurse staffing, inadequate procurement, preparation or serving of food, and inadequate quality assurance measures. In 2025 they were fined almost $50k for resident abuse. Sad to see this in a town with such an excellent health care system. If you're considering a care facility always be sure to do some research.

I stayed here for skilled care while recovering from a broken femur. It is an older facility (thus the 4 stars) but I felt very well cared for. They have a great, caring team!

My parents both lived here for about 4 years. Dad passed away but mom is still there. It seems like they are making some good changes and seem to be hiring more staff to fill the needs of the residents. My mom is happy there.

My father has been there for over a year, and we have not had any issues with them. The amount of staff has been an is concerning, but that has been an issue at most LTCF's. They are taking COVID-19 seriously, which is important right now!

My grandmother was there they where disrespectful to her. They made her have multiple panic attack’s and went against one of her rights. Not a good place to take loved ones to at all! The DON took her phone so we could not speak with her!
