Concord Nursing and Rehabilitation Center

I very recently went to visit my grandfather, and I was surprised to see the deplorable state this facility is in. According to the website and testimonials it seemed so nice and clean. The staff so warm and hospitable. But when I went to visit the facility over the weekend, it was dirty, it smelled moldy, the atmosphere was very depressing, the staff unprofessional, the patients looked uncared for, the staff lacked empathy. I’m very worried about my grandfathers state of rehabilitation in this place. It’s sad to imagine the type of care they are receiving when we aren’t around, the environment is not giving hospitality. I hope someone does something about the state of this place soon. Because if nothing changes, we’re going to have to find him a more suitable rehabilitation facility for h

I brought clothes to my mother 2 weeks ago and for whatever reason my mother still hasn't received it. I keep getting the run around while my mom is wearing other patients clothes or the same dirty clothes every time I go see her. (4x this week). It is frustrating entrusting them with her care and no real regard has been taken. I can't wait to get her out! Filing a complaint asap. If you care for your loved ones do not bring them here! They are treated worse than prisoners!

My grandma was at this rehab center from January to May after a leg amputation due to bad blood circulation. Since this was our first time admitting anyone from our family to rehab we just disregarded the reviews since we needed my grandma to go somewhere. This facility wasn’t that bad and most of the staff were nice but the food was nooottt good. See the photos, this was a daily reoccurrence with her meals and when speaking with the dietician he would just say she needs carbohydrates for since she needs to gain muscles. You can be the judge of the meals. I also disliked we would never get updates after medical visits, I would have to call the social worker to request the nurse manager to provide the after visit summary. My name is Maralin Tirado and I’ve been at your facility since 3/14. The ambience, all the workers from the cleaning experts to the nurses, to the nurses aid are fabulous and beautifully trained. They’ve made me feel so welcomed and safe and “I APPRECIATE YOU ALL”. I wish I would’ve known about this place before I went to a horrible place with the saddest experience. My whole room is cleaned from top to bottom every day and my bed is made every day. “THANK YOU”…. May God bless you all, you guys are a perfect 10!!!💯💯💯💯💯FOR THE RECORD ALL YOUR EMPLOYEES ON THE 2nd floor DESERVE A RAISE FOR THE EXCELLENT AND ON GOING CARE AND BEYOND THAT THEY PROVIDE TO YOUR RESIDENTS!!!💯💯💯💯💯
