Bartram Crossing Skilled Nursing - Brooks Rehabilitation

I spent 17 days here in Rehabilitation. The nurses, CNA, doctors were excellent. Even the people who came and got your menu order were very friendly and helpful. I can't say enough about the quality of the OT and PT staff. Here are two of my favorites Arryn and Suzanne saying good bye when I went home.

Janet is truly a gem! She’s such a hardworking and dedicated nurse assistant, and it shows in everything she does. Her kind spirit, gentle care, and positive energy make such a difference. I always feel at ease and well taken care of when she’s around. It’s such a joy seeing her thank you, Janet, for all that you do!!

I brought my grandfather to this skilled nursing facility after he suffered a stroke, and unfortunately, our experience was horrible. During his stay, my grandfather fell three times—once with the CNA,, once during physical therapy, and once under a nurse's supervision. These incidents made me question the safety and quality of care provided. I do want to acknowledge and thank two CNAs, Elizabeth and Sabrina, who were both exceptionally kind and helpful to my grandfather. Their compassion stood out amidst an otherwise disappointing experience. However, the case management services, especially from Armando, were abysmal. I was extremely hopeful when I sent my mom here after she was hit by a car…While she was there I do believe she had good care which is the only reason for 2 stars. My issue is I think they tried to get her out of there and home as quickly as possible because that’s their goal and I think they want that to help their numbers or something. Besides that the communication was horrible. It was absolutely impossible to get updates on my mom. They do have an app but they took down whatever email my mom gave them which if someone has a traumatic brain injury and can’t even tell you where they are why do you think she would know her email? Apparently after they set the portal up with one email it can not be changed I tried contacting the case manager as well as whatever email was in the website and
