Azalea health and rehab has the most amazing staff very well bedside manner, compassionate, they are over all a very well staffed team. The director Alisha goes above and beyond and is always so kind when her help is needed, the HR department is always helpful making sure my answer for anything and everything are always answered and my needs are met. My name is Cleo Williamson Jr. I am 83 year old and I was born right here in Wilmington, NC. I would like to say a few words about my visit and stay Azalea. I have had the opportunity to stay at the facility 3 times in the last five years. Twice for 90 days and once for 30 days. Excellent service by the staff, the food was good. The cleaning was excellent, and always had time to help you. I would give the facility a 4 star rating and the rehab staff a 5 star. If I had to go back again. It will be with Azalea Rehab. Everything i can say about this is excellent. May God to continue to bless them as they continue to serve the public.

The facility has serious cleanliness and pest concerns, with cockroaches and bed bugs observed. Supplies, food service, and general hygiene are inconsistent, which highly impacts patient care. Employees obviously not there to help, call bells will ring for forever. I encourage anyone considering this facility to observe really good and ask questions about safety, hygiene, and staffing. I wouldn’t let this place watch my pet fish. If you do have family here, visit and pop in as often as you can and watch how the staff interacts! Ring the call bell and see how fast of help you will get.

This review won’t allow me to say all I want to…. PLEASE DO NOT GO To Azalea in Wilmington. I promised my mother on her deathbed that I would warn the public. Please! I have a Masters Degree in healthcare with experience in long term/skilled nursing but I can’t say much on this review for fear of retaliation. I will be contacting their Cooperate office compliance line( but you know they have a legal team Much bigger than me) and whoever the State inspectors are . I’m not from North Carolina, so I am not sure who I contact but I will. I’m trying to save your loved one from pain and neglect,

My seventy four year old grandmother has only been here for about two months?Which I did not consent my own, but the government is forcing her to stay here for healthier purposes so they think. She has complained to me so many times that she is not getting changed on correct timing.Whenever she dings the bell, that the food is definitely not good and inedible.So my cousin and I and my mother try to get her food almost every day whenever we can.But mind you we all work. I am also a retired rn and cna now back to DCW due to these concerns of even hospitals, not treating people right in neglecting everyone around, it is not just a choice that she chose to stay,again government is making her stay in a rehab versus our homes. Back to her issues she does not like it here and is constantly tellin
