Avina of Mayville

Crossroads/Avina was such a godsend for my uncle's care. In 2025, he required skilled nursing rehabilitation following an extensive hospital stay. The staff were gracious, helpful, and communicative, and my uncle was happy with his stay. Unfortunately, he needed another stay recently, and I actually requested Crossroads/Avina, because our experiences with other facilities were so poor, and our experience with Crossroads/Avina in 2025 was excellent. My uncle is home now, following his 2026 stay, and I still can't say enough about this facility and its staff. 10/10, even though I can only give five stars. Side note: the daily rate was far less than I anticipated, and we had to private pay due to circumstances.

My Father was here in 2025 for rehab. Rehab did an amazing job and the staff went above and beyond for him. I would highly recommend Avina of Mayville.

This place provides awful care. Catheters aren’t emptied resulting in frequent utis. Residents often have 2 briefs put on them so CNAs can avoid changing them. Multiple residents were left for over 24 hours in their own feces. CNAs are forced to operate lofts unsafely due to low staff. Many falls, skin infections and issues.

My family member was well taken care of and the staff were very kind to him. They've accommodated his needs the best they can and I'm thankful for them. My only criticism would be is the food. For any patient who is healing physically, I think eating organic foods would help assist in healing the body. Using natural ingredients and unique ways of cooking would allow the body to keep going. Example would be, if someone is on a sodium strict diet; maybe using lemon juice as an alternative to salt. Instead of using sugar, stevia would be another alternative. The food lacked creative, flavor, and seemed unappealing. Overall, I am happy how the care was and I would recommend this place again. Thank you again.

It was awful. I had a stroke and my husband was told i would get 4 hours a day of rehab which is essential after a stroke. I DIDNT get nearly the rehab i needed and my long term recovery has been affected because of it. Understaffed AWFUL food. I would NEVER recommend this place to anyone. Kept me until insurance was up then told me youre out . My husband was trying to get me transfered to another facility and Crossroad wouldnt help with any of it. If you care about your family member Dont Send them here.
