During the final 21 days of my cousin’s life—who was more like a brother to me and whom I had the honor of serving as caretaker—I visited him daily at Harts Harbor Rehabilitative/Nursing Home Center while he received care and rehabilitative services. He had been diagnosed with terminal cancer that had metastasized to his bones. Our initial hope was that rehabilitation would help him regain some physical strength and independence so that he could return home. Sadly, he transitioned on February 4, 2026. Although our outcome was not what we prayed for, I cannot express enough gratitude for the exceptional care, compassion, and professionalism demonstrated by the staff at Harts Harbor. I would leave less than a half as a star if I could this place was supposed to take care of me and said they're Wound Care Unit let me turn to gangrene they refused medical service would not let me call 911 and refused to call a doctor and to see me immediately when it started to smell. Once a doctor was brought in to see me he sent me immediately to the emergency room where are they told me upon seeing me that I was definitely going to lose my left foot possibly my left leg they would try to save my right leg to which no avail they were able to I lost both legs I filed lawsuit to which they filed bankruptcy I am not the only one that filed laws filed a lawsuit three others did so as well and we are left in a medical situation and to which we cannot afford to be taken care of appropriately

My sister has been trapped there for years. I've tried n tried to get her outta there. They have blocked my efforts every time. I've reported her neglect n abuse with no results. Went again today to see she's neglected again. So wish again I could find a better place here n Jax for her to go. She's unkempt and neglected again today. I reported it again. Don't let your loved one go there if u really care about them. A lot of neglecte going on there

What was years past is not what Harts Harbor is today. I have a loved one in this facility and could not receive better care. The staff beginning at front door are welcoming and friendly to everyone entering. Staff and nurses courteous show respect to patients and visitors. The place may be old but they keep it clean and working diligently to improve the facility. They may not be a fit for everyone's case but they are the best for my family. Happy at Harts Harbor.

Avoid this place at all costs; if you have the option to send your loved one anywhere else, please consider it. Harts Harbor has proactively contributed to the detriment of my mentally disabled mother's health. She arrived from the hospital after having an infection in her recently amputated leg. During her 4-5 day stay, she did not have the wound cleaned or rebandaged, she was not provided medicine prescribed by the hospital, and she was given meals she was allergic to multiple times a day. She is unable to use the bathroom on her own and was forced to relieve herself in bed and left like this for hours with the bedsheets never changing. Harts Harbor did not want to address her basic needs and eventually sent her via ambulance to a hospital that had no record or information on why she wa
