Achieve Rehab and Nursing Facility

My mother has been here twice for short term rehab after knee replacements. She received excellent care each time.

My husband is quite comfortable at Achieve. The staff at Achieve are very attentive, responsive, friendly, and professional... this is a pleasure after being in another facility in the county for months. There, he cried every day because he was being neglected, and the staff acted as tho they didn't want to help the residents. I am so happy we took a chance with Achieve Rehab and Nursing Home. It is a pleasant environment. The residents are their priority, not meant to feel that they are a burden. They feel more like being cared for by family! I recommend Achieve for families looking for a place for their loved one to receive professional care in Sullivan County.

Very accommodating trying their best to help all patients even the ones that live there long-term. The staff has very much improved they are striving for the better . Myself and my family support them in their efforts this place is definitely up and coming. So if you'll find yourself in need of a facility please consider Achieve.

This place should be shut down I've seen cockaroaches in the hallways and in rooms. The staff are over worked and short handed daily. There is no quality of care and is more worried about saving money and cutting corners that patients and staff are at risk of injury or worse. My friends dad is there and he had a broken arm and no one knew. Nobody knows how long his arm was broken. No pain meds nothing. This is a horrible place please don't place a friend or family member here and pass the word.

I've been a patient here for several weeks now. I couldn't walk when I arrived after brain surgery and had severe vestibular problems. The team here knew exactly what to do (in succession) Absolutely everyone here is knowledgable, professional and responsive. The staff is friendly and the facility very nice and clean. The Occupational Therapy, Physical Therapy, and Speech Therapy staff are beyond compare! Do not hesitate to consider this facility for services or long term placement. W. Allen 🤗
