My dad is there now and has been for almost 2 weeks. I went to many places before making a decision & I'm glad I went with my gut. My dad is an active man and is much younger than this actual age & just wants to heal and go home, but he has a broken leg ( 3 places ) and can’t move leg much at all, I’m mentioning that because the physical therapists are fantastic, they work with him 2x a day plus he gets occupational therapy. He also tells me the food is very good. I visit him everyday and everyone is nice. ( photo of my dad’s view, beautiful outside, we sit outside all the day & he love the breeze in his room ) even if a person doesn’t get the view, it’s okay because it’s very close to every room. We are definitely happy!

Easy check in, bathrooms at the front door after a 3 hr trip very nice hahaha! Beautiful covered outdoor area to sit in at the center of the facility with citrus trees & bird of paradise and the area is in clear view of staff at all times. A sidewalk area around the patio which is great for a few laps. Also located in a beautiful neighborhood! Very clean & the staff from the front door, to the attending team down to housekeeping, were very warm, friendly, kind, professional, very busy & patient with all types of personalities was my observation. My only con were the residents were parked in their wheel chairs right smack in the center of the hallways as opposed off to the side where they would not obstruct the flow of traffic, as this is a very busy facility. My dad has been twice in TOPA the past year. I have come to know staff on a personal level. You have all been very caring, very helpful, very friendly and have taken very good care of my dad. He came in with bed sores which they were able to heal. They were able to improve his appetite. They were able to improve his overall condition in a short amount of time.So he was able to go back to the assisted living together with my mom. His mental and physical health significantly improved. I am very involved in my dad's healthcare. And this facility has embraced my involvement answered all my questions and stepped up to provide the level of care that I expected. I can recommend them for your loved ones needing extra care.

My grandma was admitted to Thousand Oaks Post Acute for rehab, and I’m truly grateful for the care she received. The staff were attentive, kind, and always made sure she was comfortable. The rehab team was amazing — they helped her make great progress and treated her with patience and respect. The facility was clean, welcoming, and communication with our family was excellent. I can tell they genuinely care about their patients. Thank you to everyone at Thousand Oaks Post Acute for helping my grandma recover and for providing such compassionate care.

The facility is lovely and when the sun is shining it’s so pleasant in the courtyard. All the nursing staff are very pleasant, knowledgeable, efficient, and caring.There was a variety of food and very enjoyable. The dietary department makes an effort to provide your preferences. Also, activities department does a great job with keeping the residents occupied. I also was able to attend a car show they hosted in their very own parking lot. I hope to never go back to a skilled nursing facility but if I need to it would definitely only be at Thousand Oaks Post Acute. Special thank you to my CNAs Cesar, Barbara, Dean, and Teresa. Also, to my physical therapist Bryan and Noa. Lastly, I would like to thank Chelsea for always willing to help with any aspect to the care.
