The Pavilion at Creekwood

I would leave 0 stars if I could. My mother and I entrusted this facility with my father’s rehabilitation after he fell, broke his arm, and injured his knee. He was transferred here on November 1st from Mansfield Methodist Medical Center, which had recommended the facility. From the beginning, we raised our primary concern: he needed to be monitored closely and placed near the nurse’s station due to his high fall risk. Instead, he was placed in the very last room of the hallway, the furthest point from the nurses’ desk. Room Conditions The room was not clean. The floors were sticky, surfaces were dusty, and a musty smell came from the air unit. This was concerning, but unfortunately it was only the beginning. Care Concerns Throughout his stay, we observed repeated issues with basic care:

You would think that a facility as beautiful as the pavilion is, in mansfield would be a great experience but unfortunately, it's the worst! Our experience was on the short term side, the rehab. The woman who gave a tour is nothing but a lying salesperson. Anything to book another family to the facility.Nothing that they said they did. The nurses, charge nurse, patient advocates/caseworker and team are a joke. They care only about hours and getting money and not about the patients or their care. Every time we told them something, they never communicated it to each other, completely unprofessional. Unfortunately, you can not advocate to much for your family member because you do not know what the care will be like when you leave. He was always dirty, face unclean, room musty, bath

I think this place is very good. The worker is very kind and friendly house keeper are good clean. they take care my father very good. my father has been here a week for therapy he has broken hip he has therapy 5 times a week The therapist are good too. Very nice place

One night was one too many My mom was recovering from a bacterial UTI and was too weak to move back home. In addition, she contracted covid almost 6 weeks prior and was still testing positive. Because of that, her options were very limited for nursing homes who would accept her. I received a call in the morning from the Pavilion at Creekwood and they confirmed she would be arriving in the afternoon. She arrived at 4 PM. Despite it being a Friday, her room had not been stocked - there were no blankets, towels, hand towels, washcloths or hand soap. The bed had not been made. The gentleman who transported her from the hospital put the only sheet on the bed. She did not receive towels and a blanket until 8:30 PM. There was zero orientation. The place looks really clean.... It doesn't matter how things. Look, it's hell the doctor's a nurse's treat their patients. I've been up here every single day with my mother for over a week.And not once have I been impressed with any of them. Not just what I see with my mother, but what I see them with other patients. Most of them should not be in the medical field. The people who just assist! just helpers have a more bedside manner than anyone else.
