The Grand Rehabilitation and Nursing at Mohawk Valley

This place needs to be shut down. The whole facility smells like urine. A loved one I know who has been in rehab for 2 weeks has gone downhill since being there. Today when I arrived she was soaked in urine. This has happened more than once. The aides actually had her in the cafeteria ready to eat while she was soaked in urine. This is neglect and against the law. If they cannot provide basic care and decency to my loved one they should be shut down and held legally responsible. I did report this to the supervisor in charge. The supervisor did state that she is suppose to be toilet every 2 hours. This does not happen because family members stay at least 3 hours with no one offering here to go to the bathroom. This is also neglect. If the grand cant give the basic care and needs of the re

(2/01/2026) I am leaving this review in extreme distress and heartbreak. My mother was hospitalized here for a stomach-related medical issue described as a “distended stomach,” and I was not notified for three full days. I only found out because I went to her nursing home to visit her and was told she wasn’t there. Yesterday was already the fourth day—and it was also her birthday. I live three hours away in Rochester. Had I known my mother was hospitalized, I would have been here immediately. Instead, I lost precious time with her, on her birthday, because no one contacted me. No call to me. No call to any emergency contact. Nothing. Being kept in the dark while your loved one is hospitalized is devastating and unacceptable. I wouldn’t even give this place one star. My son was there and the neglect he experienced along with the ignorance of the nursing staff on the second floor was horrific. I was called at 1AM and was told my son had fallen out of bed snd neuro checks were being done. Within 24 hours he was incoherent and I had to force them to send him back to the hospital.

My mother has been there a couple of months and has had numerous trips to outside hospital ER's simply put as poor care. She has been found numerous times sitting in soiled clothing. Facility is not staffed adequately and some aids just care about a paycheck rather than providing quality care with a small bit of personal care. I would not recommend this facility to anyone that truly cares about your family

After the last four years, the Grand in Ilion finally hired an exceptional administrator. One who actually cares about our loved ones. Casey, (the new administrator) you are amazing! My mother-in-law has been at the Grand for a while now. Lately she hasn’t been eating much. We expressed our concern. Casey went to her room, sat with her to figure out what she can do. My mother-in-law mentioned spaghetti and meatballs. Casey actually ordered from a restaurant exactly that, and she didn’t stop there, Casey actually fed my mother-in-law too!! (Who ate 1/2 the order). That evening we received a very pleasant phone call from Casey. I cannot express how thankful we are for caring so much. You are one of the best in our eyes!
