I arrived at the facility and discovered my loved one with a significant bruise to her forehead and a black eye (photographs available). I was informed that the injury reportedly occurred around 6:00 a.m. that morning. However, no incident report had been filed and no family notification had been made. It was not until approximately 5:00 p.m., when I arrived in person, that the injury was discovered by family.The LPN on duty was unaware of what had occurred, and no staff member initially had clear knowledge of the incident. I was later told that a staff member working around 6:00 a.m. witnessed the resident bump her head on a door but did not know how to file an incident report. She reportedly informed a supervisor; however, the supervisor failed to complete the report due to being “too bu

They don’t take care of the Alzheimer’s patients. Don’t bathe them often enough. Dont even serve correct food for patients with no teeth; and they don’t cut up the food. Patients lose up to 20# before they die. Horrible place. Do not recommend. My mother past away there. Then they sent me a bill for me to personally pay over $60,000. Got an attorney who helped me get it resolved. I never signed anything to say I would pay anything.

I had to remove my prior review because I felt bad for being so vicious with my review of the care or lack of care I witnessed in this facility with my loved one. I will say it was enough to warrant me filing a grievance, which I felt was handled like it was a joke. So I felt it necessary to file a formal complaint.

Do you not have any staff, or are all your patients dead and you're afraid to address that? I have never spent more time on a phone ringing into the abyss and don't even know if the patient im looking for is even alive! I get having a busy day here and there, but for the same thing to happen for weeks... sometimes trying several times, up to a dozen times a day... no answer, get hung up on, or the famous, "please hold" transfer that goes to nowhere. Absolutely lame. Will you answer if I drive all the way out there or will it just be a wasted, and very expensive trip for no reason? I have zero trust in your care abilities at this time.

This is not a good place to put your family in. My brother in law had been there 5 months and ha been hospitalized 2 times for bed sores. He ha a bad sore that the doctors are having trouble getting it to clear up. First time he was in the hospital 8 weeks. Released back to them and in 3 weeks he is back in hospital with the sore that is worse then the first time .
