Sapphire Nursing and Rehabilitation at Goshen

My name is Kristie, My Mother was here a year ago. She needed a little encouragement that she could stand without falling when she arrived 2/3. Mine and my brothers 1st experience with having to care for her. Sharp as a tac, but getting a little unsteady. The schedule of rehab was poor. She was left to sit in her room most of the day. They do offer things to do, but lack the encouragement for the shy and elderly (stubborn). The rehab room was not sanitary. No masks or gloves with staff or visitors. Did not see anyone cleaning any of the equipment after use. No mandatory use of cleaning solutions or hand sanitizers. At 1st no roommate, and then a woman was moved out of a room down the hall to my Moms. We were definitely not told the truth of why. Mom deteriorated. Thought it was a little co

Sapphire was blessing to me this past summer. My brother in law Thomas was visiting from Florida when he developed blood clots in both legs and was hospitalized for 2 weeks. Afterward, he needed rehab, physical therapy and extended care that I was not able to provide. The Director of Admissions, Adela Adkins was a miracle worker in getting him admitted to Sapphire and making me feel confident that he would have a successful recovery. Jen - head of nursing is so compassionate and resourceful in all she does. The entire staff was wonderful and caring. I am happy to say that Thomas has totally recovered and living his best life again. I will be forever grateful for their service during this difficult time.

The facility personnel are not trustworthy. They will tell you what works for them, not what is best for the patient. I get sick every time I go to visit my loved one. My opinion, this facility is very dirty, you never get the same story twice from the staff. Doctors orders are never followed and they'll put the blame on ,"Oh I'm just a night nurse or I don't know, I just work weekends." I have 11 months of proof. I can't sleep at night thinking about all the horrible events I've witnessed, and realizing no one cares to communicate with accurate information. I will not rest until our elders are treated with respect. My suggestion, if you can, stay far away from this facility.llllbeen a horror show of lies and inconsistent information.

The Staff at Sapphire Nursing and Rehabilitation are AMAZING! The Nurses and CNA's at the facility are compassionate, caring, informative to family on updates or medical issues that should arise , well trained in all areas of the patients medical needs . They also treat the patients like they are family! During the residents good days or bad days they may have ,the staff is always there to encourage them to have a better day or share in their joy. The P.T and O.T staff at the facility are WONDERFUL ! The residents that need rehabilitation and those that come for a temporary period of time to get rehabilitation to be able to go home , look forward to those that provide the rehab. as they are knowledgeable , very friendly , encouraging , and always greets the pt. with a smile and eve

My uncle has been at this facility for about two months now and I can not say enough of good things about the kind and caring individuals that work here. The nurses, aides and physical therapists are superb! My uncle can be very difficult and they have had so much patience with him. Thank you everyone, my mom and I cannot thank you enough.
